Fort Hudson is excited to announce CARE MANAGEMENT’s newest service initiative: CHILDREN’S HEALTH HOME.
This program serves Medicaid-eligible youth, age birth to 21, with complex medical, behavioral, and/or social needs. FH Care Managers develop interventions, connecting clients with resources. Healthcare, Education, and Social Service Systems can be overwhelming. Our mission is to support navigation of those systems, providing person-centered advocacy to the youth we serve.
