Care Management

As an approved NYS Care Management Provider, we partner with community groups and New York programs, including NHTD, TBI, MLTC, and Adult and Children’s Health Home. Managing health and social needs in the community is complex—and it’s what we do best.

Our experienced team of professional Care Managers provide guidance and resources to clients and families. Through person-centered assessment, oversight, and coordination of Home and Community support for infants, adults, and elders, we build personalized plans that connect people with the right resources.

We provide:

  • Comprehensive Needs Assessments to identify areas where support is needed

  • Advocacy and connection to vital local resources

  • Referrals for financial and legal support services

  • Education, coaching, and ongoing support for caregivers

  • Regular monitoring and follow-up to ensure peace of mind

Our approach combines clinical expertise with a strong understanding of community-based programs and managed care systems. We work closely with individuals, families, and insurance carriers to ensure each person receives the right services at the right time.

Children’s Health Home Services

Fort Hudson is proud to be an approved service provider for the Children’s Health Home program, serving Medicaid-eligible youth from birth through age 21 who have complex medical, behavioral, and/or social needs.

Through this program, our dedicated Care Managers work closely with children and their families to develop individualized care plans and connect them with vital resources. We understand that navigating healthcare, education, and social service systems can be overwhelming — that’s why our mission is to provide compassionate, person-centered advocacy every step of the way.
