Fort Hudson is proud to be an approved service provider for the Children’s Health Home program, serving Medicaid-eligible youth from birth through age 21 who have complex medical, behavioral, and/or social needs.
Through this program, our dedicated Care Managers work closely with children and their families to develop individualized care plans and connect them with vital resources. Fort Hudson: Approved Provider for NHTD & TBI Medicaid Waiver Programs
Fort Hudson is an approved service provider for both the New York State Nursing Home Transition and Diversion (NHTD) Waiver and the Traumatic Brain Injury (TBI) Waiver. Through these programs, eligible individuals can choose Fort Hudson to serve as their Service Coordinator, offering trusted guidance and support throughout their care journey.
As a Service Coordinator, Fort Hudson provides:

Fort Hudson is an approved service provider for the Adult Health Home Care Management program, which offers coordinated care for individuals over the age of 18 who are living with multiple chronic health conditions or serious mental illness.
The Health Home model is a team-based, person-centered approach that brings together the individual, their healthcare providers, and family members (when appropriate) to develop a comprehensive care plan. A dedicated Health Home Care Manager serves as the central point of contact, ensuring clear communication among all team members and keeping the individual’s goals at the forefront.
This program strengthens connections to community supports and resources, while enhancing the coordination and integration of primary care and behavioral health services—ultimately improving health outcomes and quality of life for those with complex care needs.
Fort Hudson: Subcontracted Care Management Provider for Nascentia Health Options (MLTC)
Fort Hudson proudly serves as a subcontracted Care Management provider for Nascentia Health Options, a Managed Long-Term Care (MLTC) Medicaid program. This program is designed to support individuals who are chronically ill or living with disabilities and require long-term care services—such as home care or adult day programs—to remain safely and independently in their homes and communities.
Our Care Management team offers a personalized approach, providing hands-on support and in-depth knowledge of local resources to ensure each member receives the right services at the right time.
