Fort Hudson is an APPROVED service provider for Health Home Care Management through Adirondack Health Institute
A health home offers coordinated care to individuals with multiple chronic health conditions. The health home is a team-based clinical approach that includes the consumer, his or her providers, and family members, when appropriate. A Health Home Care Manager is the liaison between team members who helps to keep the focus consumer centric. The health home builds linkages to community supports and resources as well as enhances coordination and integration of primary and behavioral health care to better meet the needs of people with multiple chronic illnesses.

Managed Long Term Care (MLTC) is a Medicaid plan that helps people who are chronically ill or have disabilities, and who need health and long-term care services, such as home care or adult day care, stay in their homes and communities as long as possible.
