Fort Hudson is a NYS Approved Care Management Provider. This means that our staff provide Care Management to individuals within a ten-county region from Albany NY to Clinton County NY. We work with other community resources to arrange services for those who are part of NYS Waiver programs NHTD (nursing home transition & diversion), TBI (traumatic brain injury), Managed Long Term Care (MLTC), Adult Health Home and Child Health Home. This coordination of care allows individuals to remain at home within their community.
