Fort Hudson Nursing Center

Certified Nurse Aide Training – Do you enjoy getting to know elders in your community?  Do you have interest in healthcare, but aren’t sure how to enter the field?   Fort Hudson is looking to jumpstart your career pathway as a Certified Nurse Aide!  Currently accepting applications for the CNA Training Program through BOCES.
Class run schedule:  March 5 – May 3, 2018
Applications due by:  February 19, 2018

Director of Activities – Full Time.  Duties include: plan, prepare, and direct the day to day operations of the Activities Dept.  This includes scheduling/staffing & working with fellow staff members in facilitating a sense of community among Residents, while fostering individual interests.
Qualifications: a Bachelor of Science Degree from an accredited university in Therapeutic Recreation or health related field and a  minimum (2) years experience in long term care.  Candidate must be a Certified Therapeutic Recreation Specialist (CTRS) or an Occupational Therapist (OT).

Staff Development RN – Part Time (20 Hours per week).  Duties include: hands on inservicing for both clinical and non-clinical staff; clinical competencies; MDS assistance.  Requirements: experience in Adult Education, MDS knowledge, ANEC Certified preferred.

LPN – Part Time.  Day Shift.

Certified Nursing Assistant (CNA) Full Time  2-10PM shift.

Fort Hudson Care Management

Care Management Supervisor – Full Time.   Primary function is to supervise and support the operations of Fort Hudson’s Care Management Division.  Duties include providing direction, training, and supervision to all care managers.  This position will also fulfill all roles of a Care Manager.
Requirements: current RN license.  Must meet any eligibility criteria for Care Management roles.  Minimum of five (5) years experience as a Registered Nurse.  Two (2) or more years experience as RN in a Care Management role. Knowledge of existing services and referral processes for community based services for individuals with disabilities, or seniors.  UAS and PRI/Screen Certified preferred for appointment; required continued appointment.

Care Management/RN Care Manager – Full Time.  ESSEX/ CLINTON COUNTIES & SURROUNDING AREAS.   Full range of care management functions; including pre-enrollment assessments, care plan development, and ongoing monitoring.  Requires local travel.  Must have at least (2) years home health and/or care management experience.  UAS and PRI certification desired, but not required.

Service Coordinator – Full Time.  Work with seniors and individuals with disabilities throughout the region.  Eligible candidates must have MSW or other degree in social work, human services, or registered nursing.  Position includes writing and coordinating service plans, knowledge of local and regional community resources, and management of assigned client caseload.  Service Coordination or Care Management experience required.  NHTD or TBI Medicaid Waiver Programs experience a plus.  Competitive salary and benefit package.

 

Fort Hudson Homecare (licensed)

RN Supervisor – Full Time.  Responsibilities include client case management, staff supervision, and provide limited phone support on rotating basis.   Qualifications: highly organized and self directing; prior homecare experience desired.

Personal Care Aides (PCAs) provide essential support and services that enable older adults and individuals with disabilities to reside safely in their homes.  Aides are known by many job titles including personal assistant, direct care worker, direct support care, and in-home care providers.  These workers constitute the fastest growing occupation in the nation. RN- Clinical Support.  Part Time.   Duties includes: creation of the Plan of Care, review of the new referrals, medication reconciliation for Therapy Only cases.
Qualifications: Minimum of (1) year’s experience in a supervised setting.  Previous homecare / supervisory experience preferred. Current CPR certification.  Valid NYS Drivers License required.

RN (Registered Nurse) Case Manager – FULL TIME or PER DIEM positions available. Homecare experience preferred.  (1) year clinical experience required.  IV and wound care experience a plus.  Excellent references required.   Ability to work well with a wide range of patients throughout Warren and Washington Counties.

Speech-Language Pathologist (SLP) – Per Diem.

Occupational Therapist (OT) – Per Diem

Masters of Social Work (MSW) – Per Diem.
