Fort Hudson Nursing Center
RN Nurse Manager – Full Time, working on 40-Bed unit. A minimum of (2) years Long Term Care experience. Must have exceptional customer service skills. MDS and computer proficiency required.
Unit Assistant – Per Diem. (Every other Weekend). Provide a diverse range of activities to focused groups of Residents.
LPN – Part Time and Per Diem positions available.

Fort Hudson Care Management is GROWING!
RN Care Manager – Full Time. Wide range of care management functions; including pre-enrollment assessments, care plan development, and ongoing monitoring. Requires local travel. Must have at least (2) years home health and/or care management experience preferred. UAS and PRI certification desired, but not required.

Social Worker – Full Time. Responsible for assessing and coordinating community services for at risk adults in the community. Ideal candidate will have Care Management experience, with NHTD/TBI or Health Home experience. Bachelors Degree required.

RN (Registered Nurse) Case Manager – FULL TIME or PART TIME positions available. Homecare experience preferred. (1) year clinical experience required. IV and wound care experience a plus. Excellent references required. Ability to work well with a wide range of patients throughout Warren and Washington Counties.
