Fort Hudson Nursing Center

Clinical Nurse Liaison RN  Part Time.  Mon-Fri  with flexible hours.  Responsible for reviewing, assessing, and communicating with referred patients and hospital staff in acute care; identifying, prioritizing and developing referral sources within the assigned market.  Ensure a smooth transition from the hospital to nursing facility.  Admission screening and referral activity will extend beyond the acute care setting at times, and may include other program referral management. 

Staff Educator RN – Part Time.  (20 hours per week).  Duties include: hands on clinical inservicing of both non-clinical and clinical staff; clinical competencies, MDS assistance.

Social Worker: MEDICAL & SOCIAL DAY PROGRAM – Full Time. Seeking creative individual with experience working with the elderly, disabled and/or dementia population, within an adult day program OR community based setting. Unique opportunity to integrate medical & social model program design. Bachelor’s Degree in Human Services field required.

Unit Assistant – Per Diem.  (Every other Weekend).  Provide a diverse range of activities to focused groups of Residents.

LPN – Part Time and Per Diem positions available.

Certified Nursing Assistant (CNA) Full Time and Part Time positions available.

Dietary Aide – Assist in preparation and assembly of Resident meals, as well as the sanitation & cleanliness of work areas.

Cook – Meets the standards of the department, for providing nutritious, attractive, and tasty food items.

Fort Hudson Care Management

RN Care Manager – Full Time.  Wide range of care management functions; including pre-enrollment assessments, care plan development, and ongoing monitoring.  Requires local travel.  Must have at least (2) years home health and/or care management experience.   UAS and PRI certification desired, but not required.


Fort Hudson Homecare (licensed)

Personal Care Aides (PCAs) provide essential support and services that enable older adults and individuals with disabilities to reside safely in their homes.  Aides are known by many job titles including personal assistant, direct care worker, direct support care, and in-home care providers.  These workers constitute the fastest growing occupation in the nation. This is YOUR opportunity to enter the healthcare field as a Personal Care Aide.

Intake & Insurance Specialist – Full Time.  Registers new patients to computer system.  Identifies primary payor for services utilizing provided insurance databases; ensures authorizations are obtained by professional staff with data entry.

RN (Registered Nurse) Case Manager – FULL TIME or PER DIEM positions available. Homecare experience preferred.  (1) year clinical experience required.  IV and wound care experience a plus.  Excellent references required.   Ability to work well with a wide range of patients throughout Warren and Washington Counties.

Speech-Language Pathologist (SLP) – Per Diem.

Occupational Therapist (OT) – Per Diem

Masters of Social Work (MSW) – Per Diem.
