Fort Hudson Home & Community Services allows our team to support your needs within the comfort of your private residence.  Community based services available through Fort Hudson Homecare include Companion Care, Licensed Home Healthcare, Certified Home Health Agency, and Service Coordination. 

We understand you have unique needs, and we offer flexibility of services and hours to support you. Whether you require short-term service following hospitalization, rehabilitation, or illness; or ongoing service due to chronic condition – Fort Hudson Homecare is the best choice for you.

Our staff, comprised of Personal Companions, Personal Care Aides, Home Health Aides, Licensed Practical Nurses, Registered Nurses, Physical Therapists, Occupational Therapists, Speech & Language Pathologists, Medical Social Workers, and Service Coordinators are proud members of the Fort Hudson team. All employees undergo extensive background screening and training to ensure the most innovative approach to homecare services.

COMPANION CARE

Companion care is a perfect option for those who wish to have some supportive services at home. Services may include:

  • Meal preparation
  • Housekeeping & laundry
  • Medication and personal care cueing
  • Errands
  • Socialization and safety monitoring

LICENSED HOME HEALTHCARE

Individuals requiring ‘hands-on clinical care’ will have the benefit of companion level services, with the additional support of enhanced clinical team involvement.  Services may include:

  • Hands-on personal care & hygiene
  • Feeding & nutritional support
  • Ambulation & exercises
  • R.N. supervision & case management

CERTIFIED HOME HEALTHCARE (licensed in Warren & Washington Counties)

Certified Home Healthcare is our most extensive clinical homecare program.  Individuals with episodes of acute illness, often following a hospitalization, can receive a wide range of homecare services.  This level of care is generally covered by Medicare and other private insurances.  Services may include:

  • Diabetic & wound care
  • Medication administration & management
  • Rehabilitation therapy
  • Cancer care
  •  Pain management
  • Chronic disease management
  • Medical case management

SERVICE COORDINATION

This program is an individually designed intervention offering Comprehensive Care Management.  Fort Hudson is an approved service provider for those primarily accessing NYS Medicaid Plans, including NYS Waiver Programs (NHTD -Nursing Home Transition and Diversion and TBI -Traumatic Brain Injury).  The role of the Service Coordinator is to assist participants in development and implementation of individualized service plans – with the outcome of returning or remaining within the community environment.  Services may include:

  • professional, trained staff and clinicians for case management oversight
  • familiarity with available community-wide services for resource referral
  • assist participants to remain within their community in cost effective ways

Requirements:
