SERVICE COORDINATION
Fort Hudson is the only Washington County APPROVED service provider for the NYS Nursing Home Transition and Diversion Medicaid Waiver (NHTD). Individuals enrolled in this program can utilize Fort Hudson as a key support system for the following:
- professional, trained staff for case management, inclusive of all aspects of program
- an integral part of the comprehensive line of adult services and programs available through Fort Hudson
- education for participants and their families understanding the various options within the NHTD program
- assisting participants in making independent choices to remain residents within their community.
